In 1981, five years after defeating Apollo Creed to claim the World Heavyweight Championship title, Rocky Balboa had successfully defended his title ten times. His fame soared, bringing him both wealth and widespread recognition. However, his manager, Mickey Goldmill, grew increasingly concerned about a rising newcomer in the boxing worldJames "Clubber" Lang, who was making waves with his rapid ascent.

During an unveiling ceremony for a statue of himself on the steps of the Philadelphia Museum of Art, Rocky was prepared to announce his retirement. Before he could finish his speech, however, Clubber interrupted him. As the top-ranked contender at the time, Clubber publicly taunted Rocky, accusing him of deliberately choosing weaker opponents for his matches. With this provocation, Clubber successfully incited Rocky into accepting his challenge for a shot at the championship belt.
